Use these forms if you want to apply to volunteer as a magistrate. You must submit Appendix B with your application form. Appendix A is optional.

You should visit a magistrates’ court in general session at least once before you apply.

Read the guidance notes to help you fill in these forms.

You must submit your application form along with the relevant documents:

  • Appendix A (diversity monitoring form): this form is not mandatory, however, we encourage all applicants to complete this form to help us identify key trends in judicial diversity and any disadvantages within the recruitment process. All responses will be treated anonymously.

  • Appendix B (referees form): you must complete and submit this form with your application.
