Details
You don’t need to use this form if you:
- already get Carer’s Allowance
- get Child Benefit for a child under 12
- are a foster carer and get National Insurance credits from HM Revenue & Customs

The carer or their representative must fill in this form, not the person being looked after. They must print the form and fill it in with a pen.
