Ordering forms if you are a member of the public
If you are a member of the public and you need a DWP leaflet or claim form, you need to contact your local Jobcentre Plus.
For medical forms, (for example, a Med3 or SR1) contact your doctor or healthcare professional.
If you need a MATB1 to claim Statutory Maternity Pay or Maternity Allowance, speak to your doctor, midwife, or a healthcare professional.
Any requests from members of the public via this channel for leaflets or medical forms will not be acknowledged. Find out about benefits and financial support.
Advisors and intermediaries
