You can only apply for Income Support if either:
- you’re entitled to the severe disability premium
- you were entitled to the severe disability premium within the last month, and are still eligible for it
You may need to apply for Universal Credit instead. Check you’re eligible for Income Support before you apply.
You may not have to fill in this form. The quickest way to apply for Income Support is by phone.
